Disclosure & Consent Concerning Consumer & Investigative Consumer Reports (Basic Background Check)

This electronic form, which you should read carefully, has been provided to you because Aim Right Ministries may request Consumer Reports and/or Investigative Consumer reports from a consumer reporting agency. Aim Right Ministries will use any such report(s) solely for volunteer and employee screening purposes. Consumer Reports or Investigative Reports will be obtained from CriminalWatchDog, Inc (CriminalWatchDog.com) located at 976 Main Street, Suite 2, Waltham, MA 02451. They can be contacted at 800-515-8498. Any such reports may contain information bearing on your character, general reputation, and personal characteristics. The types of information that may be obtained include but are not limited to: social security number, criminal records checks, public court records checks, including civil, driving records, etc. The information contained in these reports may be obtained by CriminalWatchDogs.com from private or public record sources including sources identified by you in your job application or through interviews or correspondence with your past or present coworkers, neighbors, friends, associates, current or former employers, educational institutions, or other acquaintances.

I have carefully read and and understand this Disclosure and Consent form and, by my electronic signature below, consent to the release of consumer and/or investigative reports, as defined above, to Aim Right Ministries in conjunction with my association with them as an employer or volunteer. I further understand that any and all information disclosed to Aim Right Ministries by me before, during, or after my employment or tenure as a volunteer, if any, may be utilized for the purpose of obtaining the consumer reports or investigative reports requested by Aim Right Ministries. I understand that if I am an employee or volunteer with Aim Right Ministries, they may request consumer reports and/or investigative reports about me as defined above, during the course of my employment or service as a volunteer. I understand that my consent will apply throughout my employment or time of voluntary service as a volunteer, to the extent permitted by law, unless I revoke or cancel my consent by signed letter or statement to Aim Right Ministries at any time. This Disclosure and Consent From, in original, faxed, photocopied, or electronic form, will be valid for any reports that may be requested by Aim Right Ministries.

I hereby grant Aim Right Ministries, its representatives, and employees the right to take photographs and video footage of me and my property in connection Aim Right Ministries. I authorize Aim Right Ministries to use and publish the same in print and/or electronically. I agree that Aim Right Ministries may use such photographs and videos of me with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content. I have read and fully understand the terms of this release. I am 18 years of age or older and am competent to contract in my own name.

As a volunteer at Aim Right Ministries, I agree to:

Respect and Integrity: Treat all individuals with respect, kindness, and dignity regardless of age, race, gender, religion, nationality, sexual orientation, or economic status.
Professionalism: Maintain a high level of professionalism and conduct myself in a manner that reflects positively on Aim Right Ministries.
Confidentiality: Respect the confidentiality and privacy of all personal information I am exposed to.
Compliance: Follow all policies and procedures of Aim Right Ministries, including safety protocols and operational guidelines.
Communication: Communicate effectively and listen attentively, respecting the opinions and boundaries of others.
Commitment: Fulfill my commitments to the organization, including scheduled shifts and responsibilities, and provide notice if I am unable to do so.
Conflict of Interest: Avoid any activities or behavior that might create a conflict of interest for Aim Right Ministries or negatively impact its reputation.
Reporting: Report any concerns or incidents that violate the Code of Conduct or compromise the safety and well-being of participants, volunteers, or staff.

By signing this form, I agree to abide by the Volunteer Code of Conduct and understand that failure to do so may result in the termination of my volunteer status with Aim Right Ministries. I acknowledge that this volunteer role does not constitute employment, and I am not entitled to compensation or benefits.
